At the beginning of my transition, I didn't know if I would have bottom surgery. I think there were so many other more pressing issues (testosterone, chest surgery, etc.) that I was able to put off thinking about it for a long time. In the summer of 2005, I began to find myself slipping into minor depression moments and being even more self-conscious and uncomfortable about my genitals. In the fall of 2005, I began to take steps towards what I see as my final medical transitional step. I had bottom surgery, specfically metoidioplasty with urethral lengthening, scrotoplasty with testicular implants, and vaginectomy, with Dr. Miroslav Djordjevic and his Belgrade surgical team on June 19, 2007. Due to a fistula and remaining vaginal pocket, I had to have a revision with my local pediatric urologist on March 10, 2008. Also, since my testicular implants never "dropped" properly, I chose to have them replaced with tissue expanders to stretch the scrotal skin (February 4, 2009; a very small spontaneous urethral fistula on the shaft was also repaired at this time), then I had new implants inserted on April 6, 2009, all done by my local urologist and plastic surgeon. Unfortunately, despite using the expanders and replacing the implants, one of the implants still sat too high, although the surgery did create the appearance of one sac instead of two separate sides to the scrotum. Because one of the implants was still too high, I had them again replaced with tissue expanders on March 1, 2010 by my local plastic surgeon. I again stretched the skin slowly over time, this time to almost twice as much as the previous attempt. Implants were replaced on June 7, 2010, hopefully for the last and final time.
